Privacy Policy

This Privacy Policy describes how Charles Street Co LLC, doing business as ProMD Health Ft. Lauderdale (“ProMD Health Ft. Lauderdale,” “we,” “us,” or “our”), collects, uses, discloses, and protects information when you visit promdftl.com (the “Site”), submit a form on the Site, or otherwise interact with our marketing website.

This policy covers our website and marketing activities only. If you are or become a patient, your treatment records and other protected health information are governed by our separate Notice of Privacy Practices — provided at your first visit and available on request — not by this policy. See Section 11.

2. Information we collect

Information you provide directly

  • Entry-form data. If you enter the ProMD Bridal Scholarship or submit another form, we collect your name, email address, phone number, Instagram handle (optional), wedding timeframe, the treatments you’re interested in, and your consent choices.
  • Booking requests. Clicking “Book a Visit” hands you off to our scheduling partner at bookpromdhealth.com, which runs its own booking system under its own privacy practices. We do not collect your appointment details on this Site.

Information collected automatically

  • Device and usage data. Browser and device type, pages viewed, referring page, and timestamps.
  • Coarse region, for consent purposes only. When you load the Site we look up only your country and region or state code (for example “US” and “FL”), so we know which consent rules apply to you. This is resolved at the network edge, and nothing else is returned or kept — no IP address, no city, no postal code.
  • Campaign parameters. If you arrive from an ad, we read campaign identifiers from the URL (such as utm_source, utm_campaign, gclid, fbclid, msclkid) so we can tell which ad brought you here.

3. How consent works on this Site

We use Google Consent Mode v2. Until a choice is recorded, every relevant signal — ad_storage, ad_user_data, ad_personalization and analytics_storage — defaults to denied, for every visitor, with no exceptions.

  • If you are in the EU/EEA, the UK, Switzerland, Brazil, or one of 21 US states with a comprehensive consumer privacy law (California, Colorado, Connecticut, Virginia, Utah, Texas, Oregon, Montana, Delaware, Iowa, Nebraska, New Hampshire, New Jersey, Minnesota, Maryland, Tennessee, Indiana, Kentucky, Rhode Island, Washington and Nevada), you get a true opt-in: advertising and analytics cookies, pixels and tags stay off until you choose “Accept.”
  • Everywhere else, including Florida, you get a visible notice describing this tracking, and analytics and advertising begin on an implied-consent basis unless and until you opt out. Opting out takes effect immediately.

We decide which of those applies using only the country and region code described in Section 2 — never your precise location or IP address.

You can change your mind at any time. Use the control here or in the site footer to reopen the banner and change your answer. Your choice is stored in your browser for 365 days, after which we ask again.

4. Cookies, pixels and tracking technologies

We group these into three categories. You control the first two through the consent choice described above; the third is what makes the Site work and is not used for advertising.

Advertising and analytics tags are delivered through a tag manager, and analytics data is passed through our own server before reaching Google. This makes measurement more reliable and gives us more control over what is forwarded; it does not change what ultimately reaches Google once you have consented.

When you opt out, we stop the advertising and analytics categories above and actively delete the cookies they set, including Meta’s _fbp and _fbcand Google’s analytics and Ads cookies. We cannot delete cookies that Meta sets on its own facebook.com domain — those are outside this Site’s control, and you can clear them through your browser’s site-data settings or your Meta ad preferences, linked in Section 9.

5. How we use information

  • To operate, administer and improve the Site;
  • To run the ProMD Bridal Scholarship, including verifying eligibility and notifying a winner;
  • To measure advertising performance, where consent allows it;
  • To send texts or marketing messages, only if you affirmatively opted in (Section 6);
  • To meet legal obligations, prevent fraud and enforce our Terms of Use.

We do not use this Site’s marketing or analytics data to diagnose or treat anyone, or to make automated decisions about medical treatment.

6. Text message and marketing consent

If you tick the SMS consent box on an entry form, you agree to receive marketing calls and text messages, including autodialed and prerecorded messages, at the number you provided. We record the exact wording you agreed to, with a version identifier, at the moment you submit.

  • Consent is optional and is never a condition of entry or of any purchase or service.
  • Withdraw it any time by replying STOP to a message, or by contacting us.
  • Message and data rates may apply; message frequency varies.
  • We do not sell your phone number or share it for anyone else’s marketing.

7. How we share information

We do not sell personal information for money. We share it as follows:

  • Advertising and analytics platforms. Google and Meta receive the event data described in Section 4, after the consent rules in Section 3 are satisfied. Some states define “sale” or “share” broadly enough to cover this even though no money changes hands — see Section 8 for your choices.
  • Service providers. Our tag-hosting provider, our hosting and form-processing infrastructure, and a spreadsheet mirror of Bridal Scholarship entries, each acting on our instructions.
  • Booking partner. bookpromdhealth.com receives whatever you submit there directly, under its own privacy practices.
  • Legal and safety. Where required by law, subpoena or court order, or to protect our rights or anyone’s safety.
  • Business transfers. If the business is sold or reorganised, information may transfer as part of that transaction, subject to this policy or a successor.

We do not sell sensitive personal data, including data revealing a physical or mental health condition.

8. Your privacy choices and rights

Reopen the banner at any time to accept or withdraw consent.

Depending on where you live, you may also have the right to:

  • Know what personal information we hold about you and request a copy;
  • Have inaccurate information corrected;
  • Have your information deleted;
  • Opt out of the sale or sharing of your information for targeted advertising, and of profiling with legal or similarly significant effects.

To exercise any of these, contact us using the details in Section 13. We verify requests using information reasonably available to us, such as matching the email or phone number on your submission.

We do not treat browser “Do Not Track” headers as a separate mechanism; the consent banner described in Section 3 is the operative control.

10. Children’s privacy

This Site and the Bridal Scholarship are not directed to anyone under 18, and we do not knowingly collect information from children. If we learn a form was submitted by someone under 18, we delete it. A parent or guardian who believes their child submitted information should contact us.

11. Florida law and HIPAA

Florida’s Digital Bill of Rights took effect on July 1, 2024. Most of its controller obligations apply only to businesses above a $1 billion global-revenue threshold, which ProMD Health Ft. Lauderdale is well below. Independently of that threshold, Florida law requires consent before selling sensitive data, including health-condition data, and protects the data of people under 18. We honour both regardless of whether the full statute reaches us, and Florida visitors get the working notice-and-opt-out described in Section 3 even though a true opt-in is not required here.

HIPAA and medical records. If you become a patient, the treatment records, medical history and clinical photographs created during your care are protected health information governed by our Notice of Privacy Practices, not by this policy. This policy governs only marketing-website data, collected before and separately from any patient relationship.

12. Retention and security

  • Bridal Scholarship entries are retained for up to five years after the promotion closes, to support winner verification and recordkeeping, then deleted or de-identified.
  • Analytics and advertising data follow each platform’s standard retention settings, which we do not extend.
  • Your consent choice stays in your browser for 365 days.

We use reasonable administrative, technical and physical safeguards, including encrypted transmission and restricted access. No method of transmission or storage is completely secure.
